Example:

Prednisone:

Prednisone is a steroid.  It is an excellent anti-inflammatory, but does have side effects.  The most common side effects that we see include increased thirst, urination and hunger.  The patient may be more restless or pant a lot.  Uncommon side effects include inducing diabetes and congestive heart failure (more common in cats).  These effects are usually dose dependent meaning that they get worse with higher doses.  Please pay attention to the directions on the medication so that your pet is dosed appropriately.  

The document has over 33 common medications that you prescribe regularly.  Also included are common short procedures (like sedation and SQ fluids) that you do every day in practice.  These discharge instructions can also be used to train your staff on what to discuss with your clients.  These DO NOT satisfy your handout requirements for the state of California.
